NDIS plans are made for a 12 month period and are reviewed every 12 months to ensure continuity of supports or that changes are made as needed.
During your plan review, you will be able to provide feedback on what supports are working for you and how you are progressing in achieving your goals.
You will also have the opportunity to explore and set new personal goals building on your previous achievements.
Your Local Area Coordinator (eg the Brotherhood of St Laurence) or your Support Coordinator will assist you with the review process.
Your plan may also be reviewed if you experience a change in your circumstances such as your disability support needs change or your informal care arrangements change significantly.
